Can you tell us more about what firewall solution is running on your server itself? Is it all protocols/ports that are blocked or only httpd? Grep through your firewall rules with the first part of the blocked IP; check if you're using remote firewall blacklists; check that your bogon rules are up to date if it's a newish IP range.
