About two weeks ago server rebooted on its own. Then last night this was in the log at 21:04 and ever since I have been unable to keep my / partition (sda3) from becoming read-only a few minutes after reboot. I'm wondering if it's a software error causing the hardware error or a hardware error causing the software error as it appears.

Uncharted territory here I guess. The Planet's offline disk diagnostic ran from 2:00 a.m. until 7:00 a.m. and result was that drive was in fact failing and needs to be replaced.
