Hello, in all our plesk a hacker have added few lines at the foot of index pages hosted in our servers. The lines are an iframe, calling another site outside our servers, with an encripted source code.

How can this hacker put his code inside our pages?

We have some varied versions of plesk, but in all, included the last Plesk version, the hacker could enter.

Any idea?

have you upgraded your linux, apache , mysql php etc ?
what Os ? when the latest patch have been installed ?
