I upgrade my box to kernel-smp-2.4.21-27.0.1 today but after rebooting I noticed mysql was not running, I manually started it with no issues so I checked chkconfig and it was listed but I added again with chkconfig --level 345 mysql on but still on reboot it does not start automatically.
also system status in whm shows spamd, mysql, cppop, cpsrvd and apache as down but at all working as normal.

Seems to only be an issue with the dual xeon servers (mine are all 2.0ghz) and random services have issues, this one mysql is fine buy apache has issues.

you could have tried a service chkservd restart. The service manager is not instant.

run the setup utility and check the section "System services" to see if mysql is enabled there
