First off, I am accessing PHPMyAdmin through the url:

http://(server IP):2086/3rdparty/phpMyAdmin/index.php

When trying to select a database in the left hand drop down menu, I get the error

Notice: Undefined variable: hash in /usr/local/cpanel/base/3rdparty/phpMyAdmin/left.php on line 376

I have tried reloading PHPMyAdmin and changing MYSQL from 4 back to three (which included updating Apache as well).

I also am unable to do any kind of administration through the PHPMyAdmin. Even trying to add a test user doesn't work. Dropping a database doesn't work. In short, nothing works.

If anyone has any ideas, please let me know....

there must be an error on that file (left.php) line 376.

or you probably have a corrupted file.

Well I realized that, but even after a fresh reinstall????

maybe the files that you have on your computer are corrupted, try downloading it again from the website and installing it.

What I have done is delet the phpmyadmin folders and then upgrade CPanel. This pull the phpmyadmin install itself and automatically installs it. I'm not installing it from any files I have downloaded myself. So if something is corrupt, it would have to be coming from cpanel.
